Code Explanations:
Box 12 – Includes the following codes:
- C – Cost of group-term life insurance over 50,000 included in boxes 1, 3 (up to social security wage base), & 5.
- E – Elective deferrals under a section 403(b) salary reduction agreement.
- G – Elective deferrals under a section 457(b) salary reduction agreement.
- DD – Cost of employer-sponsored health coverage. The amount reported with Code DD is not taxable. This amount is for INFORMATIONAL purposes only.
Box 14 – Other
Ex Ins – Family insurance premiums paid by employees participating in the University’s Cafeteria Plan, under Section 125 of the Internal Revenue Code of 1954.
Teach – Employee deductions to the Teacher Retirement System.
What you need:
- Last pay stub for calendar year 2023.
- W-2 form.
From check stub:
To determine Federal wages
- Take Year-to-Date Total Earnings amount from check stub and add amount in Box 12 Code C from Form W-2.
- Then subtract amounts from Box 12 Codes G and E and amounts from Box 14 “Ex Ins” and “Teach”.
To determine Social security wages
- Take Year-to-Date Total Earnings amount from check stub and add Box 12 Code C from Form W-2. Then subtract Box 14 “Ex Ins” from this amount.
To determine Medicare wages and tips
- Take Year-to-Date Total Earnings amount from check stub and add Box 12 Code C from Form W-2. Then subtract Box 14 “Ex Ins” from this amount.
To determine State wages
- Take Year-to-Date Total Earnings amount from check stub and add amount in Box 12 Code C from Form W-2.
- Then subtract amounts from Box 12 Codes G and E and amount from Box 14 “Ex Ins”.
What if I don’t have my last pay stub but I have my W-2?
To determine how much you were actually paid for the year:
Take amount in Box 3 or 5. Subtract amount in Box 12 Code C. Add amount in Box 14 “Ex Ins”.
