For additional information regarding the request process, including documentation guidelines, please refer to the University Disability Support Services (DSS) and Housing and Residence Life (HRL) websites. To initiate services, all new students must schedule and participate in an intake appointment, lasting about an hour. Students also need to complete the following steps in the DSS Portal: application of request and upload a completed impact statement. The intake appointment provides an opportunity for the discussion of accommodations and services offered by this office. For ADA/Medical Accommodations in housing and residence life, students will go to the following website and complete a request form. The student will email the link to the provider for electronic completion. To request an accommodation for an Emotional Support Animal, students will go to the following website and complete a Request Form  HYPERLINK "/housing/apply-for-housing/assistance-animals.html" /housing/apply-for-housing/assistance-animals.html The students provider will complete the Verification Form located at the following website  HYPERLINK "/housing/apply-for-housing/assistance-animals.html" /housing/apply-for-housing/assistance-animals.html. The student will email the link to the provider for electronic completion. The DSS will notify the student and the Housing and Residence Life Office of the approved accommodations. The student is responsible for completing additional agreements or providing veterinarian records to HRL. At the beginning of each academic year (fall semester) students with previously approved accommodations, will schedule an appointment to meet with the DSS Staff to review accommodations and any related issues. The meeting will last approximately 30 minutes. Students are responsible for providing annual veterinarian records to HRL and other required paperwork prior to an Emotional Support Animal being allowed to return to the residence. Students are not allowed to bring an Emotional Support Animal into a residence without approval as an accommodation. Failure to request services before moving into housing may result in delay of approval. Services are not retroactive. Until students are officially registered, Disability Support Services cannot intercede. Students requesting a review of approved accommodations should complete the Accommodation Review Questionnaire in the DSS Portal and upload a completed Accommodation Review Form. Students also need to contact the DSS office to request an appointment for further discussion of accommodation review. Additional documentation may be required to support revised accommodations. DSS Staff are available by appointment to discuss accommodations, academic difficulties, or other issues. We also offer one-to-one sessions in test taking, learning style and strategies, time management, career planning, assistive technology, and self-advocacy skills.
